Present a valid passport as identification, or (in the absence of a valid passport) a birth certificate, accompanied
by a sworn affidavit (witnessed by a Notary Public or Justice of The Peace stating that they know the individual
named in the affidavit, and shown in the photograph, is the same individual named on the birth certificate). The
passport-sized photograph must also be signed by same Notary Public or Justice of the Peace.

If deemed necessary, you may opt to cancel your appointment, however, you are only eligible for a refund if your appointment is cancelled at least 24 hours in advance.
Customers will be allowed a maximum of two (2) cancellations for written and road test appointments. After the second cancellation, customers will be required to rebook the appointment and pay the prescribed fee in accordance with the Department’s policy and Traffic Act and Regulation.
